You may use the form below to place an online re-order on your contact lenses.  After submitting the form, someone from our office will will contact you within 24 hours, or on Monday if the form is submitted over the weekend.  (Our office hours are Monday, Wednesday, and Friday, 9:30 am to 5:30 pm, and 9:30 am to 7:30 pm on Tuesday and Thursday. Our Saturday hours are 9:00 am to 3:00 pm.)

You may also phone our office at (561) 391-3334 to reorder or check on the status of an order already placed online or by phone.  Items marked with a red asterisk ("*") on the form are the minimum required fields to use this form.
